Form 1000 is the nomination (attestation) form required for an applicant for a National Innovation (subclass 858) visa. The nominator attests that the applicant has an internationally recognised record of exceptional and outstanding achievement in their area of talent, is currently prominent, will be an asset to the Australian community, and can obtain employment or become independently established in that area. The completed and signed form is lodged with the applicant's visa application, and no fee may be paid to the nominator.
