What it is: Form 47A gathers information about a member of an applicant's family unit who is aged 18 or over, such as an adult child or dependent relative, whether or not that person intends to migrate. It supplements a main visa application so the Department has full details of each family member. It also includes the Australian Values Statement that migrating family members must sign.

Who needs it: It must be completed for each member of the family unit aged 18 or over, whether they are migrating or not, and is usually lodged alongside a main visa application by the applicant or that family member.

Form 47CH

Application for migration to Australia by a child

What it is: Form 47CH is the application used for a child to migrate permanently to Australia under a Child visa category, such as a dependent, adopted or orphan relative child. It captures the child's identity, family, health and character details along with sponsor information. Where the child is under 16, a parent, relative or guardian may complete and sign the form on their behalf.

Who needs it: It is completed by or on behalf of a child seeking a Child visa to join a parent or sponsor who is an Australian citizen, permanent resident or eligible New Zealand citizen.
