Form 40 is completed by a prospective sponsor to support the migration of a parent, aged dependent relative, remaining relative or carer to Australia. It records the sponsor's identity, relationship to the applicant, household and financial circumstances, and a formal sponsorship undertaking to provide support and accommodation. It is submitted together with the applicant's visa application (such as Form 47PA or 47OF).
